Coronary In-stent obstruction treated with another stent implantation
A rare but life threatening condition called Thrombosis inside a previously implanted coronary artery stent was successfully treated at SKY Hospital, Imphal. Mr. Roop Chand, 55 years old patient from Langjing, Imphal West, Manipur had been treated with a coronary artery stent in 2008 outside Manipur.
He remained well until a few days back when he developed severe chest pain and was brought as an emergency to SKY Hospital, Imphal on 3rd January, 2019 at 11:51 am with hyperacute myocardial infarction (heart attack).
The patient was immediately taken to Cath Lab via ICU. Coronary angiogram revealed thrombosis inside the previously implanted stent to his left anterior descending artery (LAD) causing 100% obstruction of blood flow through the old stent causing heart attack. The block was crossed with a hair-breath thin metal wire called “Teflon and Whisper” guide wire.
The block was dilated with coronary balloons (size: 1.5 x 10 mm & 2.5 x 10 mm). Finally a bigger German made stent (size: 2.75 x 24 mm) was inserted inside the previous stent and was expanded at a higher pressure (atmospheric pressure of 14 atm).
The final result showed free flow of blood through the newly implanted stent. This excellent result is called TIMI – III Flow. The patient started to eat and drink at 7:00 pm of the same day and mobilised the next day. He will be discharged home in 2 – 3 days.
Dr. Lairikyengbam Shyamkishore, Chief Cardiologist & Chairman of SKY Hospital expressed his satisfaction and happiness on saving the life of this patient who traditionally required heart surgery called CABG (Coronary Artery By-pass Graft) after stent failure / obstruction.
He appealed to the people of Manipur to be aware of symptom of such conditions and asked for help at specialist hospitals for appropriate treatment which will help save life.
